Case Management - Richmond, Care Coordination Department, VCU Health System

This department assists hospitalized patients with discharge planning, and outpatients with pre-hospital planning. The nurses communicate with insurance plans regarding the medical treatment plan during the hospital course for utilization management. They arrange skilled home health visits for post hospital care and medical equipment. Social Workers refer patients to financial counselors who assist patients with payment of medical expenses when patients are uninsured. Social workers assist with community resource referrals and with patient and family coping/adjustment needs.
