Description

Applications for birth certificates, and applications for declaration of paternity are available. Certified copies of death certificates are available upon request to those persons eligible to receive them. Certified copies are $12.00 per copy. Certified copies of birth certificates for birth that occur in the State of Virginia must be obtained from the Virginia Department of Health, Division of Vital Records, First Market Bank Building, 2001 Maywill Street, Richmond, VA 23230.
