Application process
Eligibility requirement are determined by FPL percentages. Screening forms are available in the office (Monday through Thursday from 9 am to 4 pm ). You must first complete the screening to make an appointment. The clinic is by appointment only Monday evenings, Tuesday morning and Wednesday evenings. Pharmacy Wednesday only from 2:00 to 7:00 pm. Eligibility Screenings held on Thursdays 9am-12pm

HELP Dental Clinic provides comprehensive dental care for low-income uninsured adults with household incomes at or below 300% of the Federal Poverty Level. Veterans who are ineligible for dental through the VA and Medicare recipients who lack a dental benefit may also qualify for care.
