Eligibility
To qualify for ACTS services one must meet the following qualifications. If you are unusre, please call and we can do a prescreening to determine eligibility. -Mental ilness diagnosis (i.e. major depressive, bipolar, schizophrenia) -Prescription for antidepressant, antipsychotic, or mood stabilizer within the last 12 months -Requires individualized training in accuiring basic living skills such as symptom management, adherence to psychiatric/medical treatment plans, development and appropriate use of social skills -Prior history of psychiatric hospitalization, crisis stabilization, psychiatric residential treatment, ICT/PACT, or TDO evaluation -Active Medicaid Insurance

8am to 4:30pm from Monday through Friday

ACTS is a community based program that provides support and training at home and in the community to individuals who have been diagnosed with a mental illness. Goals of services include training in the following areas: Medication compliance/management, Behaviors to maintain health and safety, Health, Nutrition, Good Physical Condition, ADL'S/IADL'S such as hygiene-bathing-grooming-shopping-housekeeping-accounting-food preparation-transportation, attending medical appointments, Coping Strategies, Identification of Symptoms, Social Skills, Accessing Community Resources
