Eligibility requirement are determined by FPL percentages. Screening forms are available in the office (Monday through Thursday from 9 am to 4 pm ). You must first complete the screening to make an appointment. The clinic is by appointment only Monday evenings, Tuesday morning and Wednesday evenings. Pharmacy Wednesday only from 2:00 to 7:00 pm. Eligibility Screenings held on Thursdays 9am-12pm

Medications are readily available through our in house pharmacy, professional samples, The Pharmacy Assistance Program (TPC), and Project Care specialty referrals.
