Application process

An initial intake conference will be scheduled to determine if potential participant meets eligibility criteria. Determination will be faxed or phoned within 48 hours.

Description

InnovAge Virginia PACE provides senior care that helps older adults live life on their terms in their community. All-inclusive Care for the Elderly (PACE), which helps older adults receive the healthcare and social support services they need through an interdisciplinary healthcare team. As an alternative to nursing facilities, InnovAge PACE provides nearly all medical care and social support at a center near your home.
