Pick up an enrollment application from the office. The enrollment application consists of: Student information, parent/guardian information, , medical information, the parent/guardian agreement contract, School entrance Health form for a health care professional, immunization certification and a fee payment schedule. We also need a copy of a birth certificate. (We will be glad to make a copy for you). After the application has been filled out and all forms are complete, schedule a time to come and enroll your child at Heritage. The Registration fee is due at the time of enrollment. A "Back to school night" is generally scheduled the first week of school for the Fall Program. This will give you the opportunity to receive a parent handbook, get better acquainted with your child's teacher and answer any questions you may have.
