Written application required. Please bring the items listed below to the elementary school located in your attendance zone. 1. An original certified state Birth Certificate or Birth Certificate Affidavit. 2. A certificate of physical examination from a phsician or health department. 3. An immunization certificate that meets all necessary requirements. 4. Proof of residencey, such as, current gas, water, or electric bill.
