Services include health risk assessments; routine health screening; nutritional counseling and education; health promotion; physical fitness and group exercise; music and dance-movement therapy; home injury control; mental health screening; mental health education; information on Medicare coverage; medication management; information on diagnosis, prevention, treatment and rehabilitation of age related diseases and chronic disabling conditions geriatric counseling and follow up health services counseling. Services are available to consumers unable to leave their homes.
