Must have the following in order to receive assist:I.D.Verification of income for EACH and EVERY person that lives in the house and receives ANY FORM of income.Income includes but not limited to: SSI, Social Security, employment wages, child support, Disability, TANF or other Social Service income. Copy of (RECENT) check, check stub, letter of notification from Social Service or Social Security or other OFFICIAL proof of income received with you - last 2 check stubs if paid every 2 weeks, last 4 stubs if paid weekly, etc. ALL income MUST be proven, we are unable to call Social Service or Social Security for those that come through the church during this time. It is YOUR responsibility to have the required information WITH YOU. Notice of Action if you receive SNAP (food stamps), TANF or other Social Service assistance. Birth Certificate or another Official form of I.D. on ALL children being put on Angel Tree. No child may be placed on Angel Tree without official I.D. School, Housing Contracts, or medical records may also be accepted provided they are current and include the parent's name, child's name and child's birthdate. Receipts for ANY and ALL expenses: In YOUR name with YOUR address. If you have a bill in another person's name you must supply their income or be able to prove they do not live in the household. Housing contract if you receive Housing assistance. Expenses include, but are not limited to: rent, car payment, doctor or hospital bills, heat, electricity, gas, insurance, phone, and other utilities you pay on.
