Parents must bring the following information at the time of registration: 1. Parent Photo ID 2. Child's certified original birth certificate 3. Proof of income by providing one of the following document (2020 Tax return, letter from employer indicating frequency and rate of pay for each working parent in the household, a recent paystub for all jobs worked by any parent living in home, TANF verification, SSI). 4. Proof of Residency Rental Lease, residence manager's letter on company letterhead, electric bill, water bill, gas bill, notarized letter from homeowner for families renting rooms or living with relatives or friends. Do not bring a phone bill or anyother regular mail.
