The is a person-centered evidence based services for adults 18 and older who have been diagnosed with a MHD or SMI and have experience a lapse is connection to structure and community resources. Support individuals with sustaining stability or regaining stability more independently.

Provide Mental Health Skill Building for adults this services guides individuals in living well with SMI whoever they look for them individually. We also provide Substance Use IOP 2.1 for adults. These individuals are assisted with their own person-centered wellness plan. We use evidence based practices along with some holistic tools. The both services are conducted daily/weekly. Participations and commitment is a requirement .Age range 18-35 is our target population for both services.
