Description
Provides family-centered care that involve clients with AID/HIV and their families in planning, delivery, and evaluation of health care. Facilitates communication through a team approach building on family strengths and empowers families to achieve greater adherence to the medical management regimen through emotional, social and developmental support. Offers HIV/AIDS testing and counseling, HIV management, substance abuse counseling, and educational sessions on a variety of HIV-related topics. Social workers are also available to assist with transportation, housing, food, and other community resources.
