Provides multidisciplinary evaluations for children who are suspected of having a development delay or who have a diagnosed handicapping condition; development of an Individualized Family Service Plan (IFSP) to meet outcomes desired by the family; physical, occupational and speech therapy; special instruction for infants and toddlers who are eligible under IDEA Part C. Provides a single point of entry for developmental screening, eligibility determination, service coordination and coaching of the care-giver of infants or toddlers with developmental delays or disabilities. These Early Intervention services are delivered by licensed and certified primary providers from one of the following disciplines: physical therapy, occupational therapy, speech-language pathology, licensed clinical social worker, early childhood special education, to include vision and hearing impaired services and early intervention assistants with family access to the full team of professionals as needed.
