Eligibility
dults 18 and over with a diagnosed developmental disability and with the following criteria:
Current tuberculosis screening and physical exam within one year by a physician.
Medically clear of infectious or communicable diseases.
Must not be violet, agitated, suicidal, or homicidal.
Must be Medicaid/Waiver eligible.
Enrollment is based on program availability.

Provides support to adults with intellectual and developmental disabilities. The service includes a variety of training, support and supervision in a setting which allows peer interactions and an opportunity for community and social integration. The day services also enables the individuals to improve and maintain maximum functional abilities and personal areas of interest. Creative Options utilizes person centered planning to identify the supports each individual needs to achieve their desired outcomes.
