You need the following to register your child:
Child's official, certified birth certificate*
Lease agreement AND one other proof of address
School Entrance Health Form (bring anytime before the first day of school)
Photo ID for parent or legal guardian -or- school age affidavit in accordance with state law

Please download the registration packet prior to coming to ensure you bring all necessary documents with you. Parents/guardians should register at their zoned school. Rising Kindergarten students must attend the appointment with a parent/guardian.
