Eligibility
Legally blind persons (as defined by the IRS), in good health and able to walk several miles daily at a fairly brisk pace.

8am to 4pm from Monday through Friday

Application process
Applicant completes first and last page of application, while physician must complete second and third pages. Assignment cannot be made until receipt of completed application.

Helps the visually impaired enhance their quality of life by providing the means for safe and independent travel. Provides leader dogs to individuals whose vision is impaired to the extent of legal blindness as defined by the IRS. Referral Policy: Applicant completes first and last page of application and physician must complete the second and third pages. Assignment cannot be made until receipt of completed application. An admissions committee evaluates the application and upon final acceptance, the applicant is notified of definite assignment. There is no charge for the leader dog or any part of the 4-week training program. Applicants must undergo the 4-week program with their assigned dog at the Michigan facility. Students live at the facility under constant supervision of competent instructors.
