Eligibility

Adults age 18 and older who are experiencing mental health symptoms that require a structured Partial Hospitalization Program level of care may be eligible. Individuals may be referred following psychiatric hospitalization, crisis stabilization, emergency services, outpatient providers, community agencies, or self/family referral. Clients must be medically and psychiatrically appropriate for PHP, able to participate in group-based services, and not require 24-hour inpatient care at the time of admission. Admission is based on clinical screening, diagnosis, medical necessity, safety assessment, insurance authorization when required, and program availability.

Application process

Referrals may be submitted by hospitals, crisis stabilization programs, outpatient providers, community agencies, family members, or the individual seeking services. After a referral is received, Gifted Ambition Health completes an initial screening to review presenting needs, diagnosis, safety concerns, medical necessity, insurance/authorization requirements, and program availability. If appropriate for PHP, the individual will complete intake paperwork, consent forms, assessment, safety planning, and treatment planning. Admission is based on clinical appropriateness, ability to safely participate in PHP services, and authorization approval when required.

Fees and payment requirements vary based on insurance coverage, health plan authorization, and service eligibility. Gifted Ambition Health, LLC accepts eligible referrals for Partial Hospitalization Program services and will review insurance information during the intake process.
