Developmental Disabilities (DD) Support Coordination provides individuals and families with the resources and support they need to lead happy, healthy, and fulfilling lives in their communities. Support Coordinators are CSB staff who help individuals with Intellectual and Developmental disabilities identify their strengths, needs, and preferences through person-centered planning so they can get the right help that is needed. The primary activities of the program are as follows: Identifying and reaching out to potential consumers assessing needs and planning services; Linking the individual to services and supports; Assisting the person directly to locate, develop, or obtain needed services and resources; Coordinating services with other Community Services Boards; Enhancing community integration; Making collateral contacts; Monitoring service delivery; Advocating for individuals in response to their changing needs; and Assuring service authorization of Medicaid Waiver services
