Description

Case Management is a service that is responsive and flexible to the needs of each child and family. Each child and his/her family are assisted in gaining timely access to needed medical, psychiatric, therapeutic, social, educational and other supports. The core of the case management process is teamwork and trust. Case Managers work in partnership with families in order to develop goals that will enhance a child’s potential and quality of life in the community. Case managers assess the child’s needs, develop plans to meet those needs, help the family find services near where they live, navigate school and educational services, monitor the child’s progress, provide follow-up with family to ensure needs are met, and advocate for services and supports.

Provides an agency of local government, that offers mental health, substance use, and developmental disability services where they are needed, in the local community. Region Ten has offices and facilities in the City of Charlottesville and in the counties of Albemarle, Fluvanna, Greene, Louisa and Nelson; making programming available to all children, adults and families living in the communities we serve.
