Description
Developmental Disability Support Coordination (DDSC) team serves several roles. Anyone, any age, who is diagnosed with an intellectual or other developmental delay/disability (DD) can contact us for a DD Waiver screening to determine eligibility for DD waiver funding. DD waiver funding covers expenses related to a variety of home and community supports. Support coordinators can assist with assessing a person's needs and linking, referring, and coordinating services to support people seeking services.
