Application process

Adults: Must contact Culpeper Career Center (Sam's Place) at the Culpeper Department of Human Services. The phone number is 540 727-1055 x406 or mail to Culpeper Career Center, 219 East Davis Street, Suite 100, Culpeper, VA 22701. Hearing aids use the same process. Children: Must contact any of the public school nurses for referral to the Board of Directors. Repeat applications can only be approved once every two years.

Adults may get free eye exams and glasses by contacting the Culpeper County Social Services Department, to get an appointment with George Randolph to complete the application to see if they meet the needs requirement. this application will be sent to us for a decision. If an adult needs a hearing aid the same process is followed.
