Application process

Telephone referral accepted. Appointment required. Medical referral required for Medicaid/Medicare, private insurance patients wishing to use those benefits. Referrals by physicians, healthcare providers, home health agencies, nursing homes, Disability Determination Services, the Veterans Administration, local schools, daycare facilities, and self-referrals.
