Offers Free Community Health Clinics. Community Health Workers will be onsite to assist clients with completing forms, translation, and help the MAP Clinic staff in providing information and referrals, upon request. Services are for people without any health insurance. Services Offered: School entry and youth athletic physicals (parents must be present for children 18 years and under to sign consent form) General health physicals Screenings: Hypertension, Diabetes, Asthma, Cholesterol Pregnancy testing (by Urinalysis only) Vision Screenings Hearing Screenings Acute primary care visits: Sore throat, congestion, fever, ear pain, sinus pain, cough, flu symptoms, GI symptoms, and other non-chronic illnesses.
