Eligibility
  • Resident of Virginia- Under the age of 21- Diagnosed with a long-term physical condition that is expected to last 12 months or more and this condition requires health care beyond that typically expected for children of the same age.
Hours
8am to 4:30pm from Monday through Friday

Anyone can make a referral (phone, email, website). Application assistance provided to families.
