Application process
1. Obtain inquiries and referrals from case managers, guardians, social workers, or other relevant agencies.
2. Inquire about the members service plan dates( VERY Important)
3. Email Client Screening and Referral Application to referral source
4. Once Client Screening and Referral Application is obtained request for documentation listed in Number 4 (if we decide to move forward with providing services)
5. Obtain SIS, RAT, VIDES, psychological evaluations, hospitalizations etc. from case manager
6. Schedule treatment team meeting with individual, Guardian (if applicable) case manager, and myself.
