Individuals between the ages of five and eighteen years old. Eligible individuals must have a DSM-V, Axis II, MR diagnosis and must meet at least two of the following eligibility criteria: have difficulty establishing or maintaining normal relationships; exhibits inappropriate behaviors that repeated interventions by mental health, social services and or the judicial system are necessary; display an inability to recognize personal danger or exhibits socially inappropriate behaviors to such a degree that they are at risk of an out-of-home placement; the individual is at risk of being removed from the home; or the individual is being transitioned home as a result of an out-of-home placement.

Description

Provide supportive and comprehensive intensive- in- home services to at-risk individuals and families in an effort to: stabilize the family and maintain the home environment; prevent an out of home placement; enable the individual to live at home with the necessary skills to ensure that home is a safe and successful placement; establish and work on identified treatment goals; and assist the family in becoming aware of and utilizing community resources. Intensive in-home services may include assessment, service coordination, crisis management, counseling and skills acquisition designed to assist the individual in interacting appropriately within the context and demands of the home environment. Supportive services are provided to help the family build skills for coping with the individual's disorder. These services must include individual and family sessions by a Licensed Mental Health Professional, behavior management, problem-solving, communications, self-help, activities of daily living, instrumental activities of daily living, skills acquisition, and independent living skills for the individual, and parenting skills are often included. Crisis response is an integral part of in-home services. Services may also include assistance in household management, as related to effective services for the individual. All of the services are flexible and tailored to each individual's needs and all must be available 24 hours a day, seven days a week. Services have a holistic approach and involve working in collaboration with the community to access and coordinate a full range of supports and services.

A private for-profit comprehensive community based mental health organization licensed with the Virginia Department of Behavioral Health and Developmental Services as well as a credentialed Medicaid service provider with most Managed Care Organizations. In addition, we are recipients of Comprehensive Services Act funding for adolescent and adult services.
