Eligibility

Active Medicaid with a selected MCO, a verified diagnosis of a substance-related and addictive disorder, completion of a comprehensive assessment, relative stability in daily functioning, participate can not be in a active withdraw.

Application process

Freedom of choice form, consent to services form, records release form will be completed, our wellness strategist will conduct a needs assessment, an authorization is sent to MCO for approval 7-14 day turn around

Description

Substance use group therapy, individual therapy, evidence based skills training with some holistic approaches based on a person centered treatment

RestoreMe M/H B/H Services, LLC.

Provide Mental Health Skill Building for adults this services guides individuals in living well with SMI whoever they look for them individually. We also provide Substance Use IOP 2.1 for adults. These individuals are assisted with their own person-centered wellness plan. We use evidence based practices along with some holistic tools. The both services are conducted daily/weekly. Participations and commitment is a requirement .Age range 18-35 is our target population for both services.
