Case/Care Management - Williamsburg, Parents As Teachers, Child Development Resources

Helps at-risk families with young children (prenatal to age 6 years) to access health care and to promote child development and family well-being. A nurse and family consultant team approach helps families secure access to a medical home for their young children; conduct child health and developmental screening and home safety and family risk assessments; provide education on health, nutrition, parenting, and other family needs; and coordinate social services such as housing and employment through case management.
