Vocational rehabilitation services are provided to eligible individuals under an employment plan called an Individualized Plan for Employment. This plan identifies a vocational goal, the services and strategies that will be needed for the individual to reach that goal, the providers and sponsors of identified services, and a timetable for achieving the vocational goal. Each vocational rehabilitation consumer is a candidate for job placement assistance which involves the degree of intervention needed to identify an appropriate job and coordinate the removal of barriers that stand in the way of the customer performing that job. The vocational rehabilitation program also works with local education agencies to facilitate the transition of students with disabilities to post-secondary opportunities that lead to employment outcomes. In addition, the vocational rehabilitation program collaborates with service providers and other agencies serving the same customers to coordinate services to customers, to maximize referral mechanisms to support people with disabilities.
